Foundation Donor Wall
This philanthropy wall exists to honor those who have given of their treasure to make a difference in saving and changing peoples lives in our community.
The donor wall is divided into two unique areas – the Cumulative Giving area and an area devoted to current giving.
The Cumulative Giving area represents the total history of giving by a donor to-date. As philanthropists continue to give, their history of good deeds will grow and they will be added or moved to new levels on this living tribute to philanthropy.
The Current Giving area of the Donor wall is devoted to recognizing those who are presently supporting the new Cardiovascular Center. Giving clubs have been established to recognize gifts and/or pledges. More information is available on our Giving Clubs under Giving Opportunities.
Donor Bill of Rights
To be informed of the organization’s mission, of the way the organization intends to use donated resources and of its capacity to use donations effectively for their intended purposes.
To be informed of the identity of those serving on the organization’s governing board, and to expect the board to exercise prudent judgment in its stewardship responsibilities.
To have access to the organization’s most recent financial statement.
To be assured their gifts will be used for purposes for which they were given.
To receive appropriate acknowledgment and recognition.
To be assured information about their donations is handled with respect and confidentiality to extent provided by law.
To expect all relationships with individuals representing organizations of interest to donor will be professional in nature.
To be informed whether those seeking donations are volunteers, employees of the organization or hired solicitors.
To have the opportunity for their names to be deleted from mailing lists that an organization may intend to share.
To feel free to ask questions when making a donation and to receive prompt, truthful and forthright answers.
Donor Recognition Policy
PHILOSOPHY:
To ensure that benefactors of Florida Hospital Zephyrhills that donate to the Foundation receive recognition that is warm, appropriate, equitable, and consistent; to convey the attitude that all gifts are important, that nurturing each donor is the business of the hospital and the Foundation, and that a monetary gift is never more important than individual relationships.
OBJECTIVES:
• Cultivate relationships with existing donors
• Encourage existing donors to reach higher levels of giving.
• Attract new donors.
• Ensure that one clearly stated policy of recognition is applied at all times.

DONOR WALL:
All donors with contributions will be recognized on the FHZ Giving Wall, located in the main lobby of the hospital.
• Reasons for Community Wall:
o To publicly thank and honor donors
o To encourage and excite others
o To create a community spirit
• Listing of Giving Categories and Names:
o Donors contributing $7,500 and greater will have their names displayed on an engraved wall plaque at the appropriate donor level.
o The wording for each gift should be limited to the name of the donor(s).
o The donor wall reflects cumulative totals updated annually.
PLAQUE OPPORTUNITIES AND CONDITIONS:
It is the policy of the Foundation, in conjunction with the Hospital, that new plaques will be placed throughout the facility as deemed in the best interest of the Hospital and Foundation. Amounts apply to the following:
A) OPPORTUNITIES:
• Patient Rooms: Patient rooms may be dedicated from time to time where previously determined by the Board or upon consideration by the Board of Directors.
• Equipment Naming: See “Patient Rooms.”
• Naming Rights: See “Naming Opportunities.”
• Departments: Same criteria as patient rooms other than monetary value.
• Art Work: When a gift of art is deemed appropriate to be hung in the facility it may be “plaqued” in order for visitors to appreciate its significance.
B) CONDITIONS:
1) Plaques that have previously been designated for an area will remain in place as long as the location is appropriate. See “Changes, Renovations, and Additions.”
2) All plaques will be categorized and registered in a log with an explanation of why they exist.
3) All plaques are subject to being retired to a common area for recognition, i.e. archive room. The information in the log will be updated to reflect these changes.
4) No changes will be made to plaques from businesses, organizations, and/or persons due to acquisitions, mergers, and/or name changes.
NAMING OPPORTUNITIES:
Named gift opportunities recommended by the Foundation shall be available for donations of $100,000 or more. This policy shall allow for flexibility as required by special fundraising initiatives. Identification of named gift opportunities will be considered on a project-by-project basis.
A) A letter of acknowledgement and intent must be signed by the donor and the Foundation.
B) Donors may use their gifts to honor a loved one, a friend or business associate. Named gift opportunities may be used to designate the interest and contributions of a family, business, foundation or organization.
C) The value placed on naming opportunities is based on visibility, desirability, and construction costs or a compilation of. Should changes take place in construction plans and affect selected areas or projects, donors will have the opportunity to make alternate choices.
D) Named gifts will be recognized in an appropriate manner in consultation with each donor.
E) Gifts from more than one donor may be considered to meet the established value of a named gift.
F) Once naming values have been decided, the Foundation has the authority to offer these areas for consideration to a potential donor.
G) To authorize the naming of buildings, major wings or departments, the Foundation will contact the Hospital CEO with the expectation the Foundation will give the potential donor an answer within 24 hours, thus allowing for protection of confidentiality of the consideration.
H) Following a commitment by the donor, the Foundation will communicate in written form detailing the recognition the donor will receive.
I) Factors to consider in naming areas:
a. Amount of Gift
b. Public Visibility of Area
c. Length of Recognition Time
These factors should allow for some flexibility in incorporating the above policies in order to accommodate institutional needs and donor capability of providing that need.
The Foundation Board reserves the right to withdraw recognition and naming agreements should such withdrawal be deemed in the best interest of the hospital and foundation.
